Request and Authorization to Disclose Client Information

To grant AEGIS Financial Staff the ability to communicate about a client’s financial or legal situations, obtain estate planning documents or to receive a copy of a tax return with the client’s third party professional partners, such as an Attorney and an Accountant, the client needs to complete and sign the Request and Authorization to Disclose Client Information form.

By signing the form, the account owner authorizes and directs Raymond James to provide certain non-public, confidential, personally identifiable account information including, but not limited to, client demographics, transactions, security pricing, holdings, notifications of corporate actions, trust documents, wills, tax information, statements, and trade confirmations (“Transactional Data”) relating to Account Owner’s accounts (listed above) (collectively, the “Accounts”), to the above listed parties (collectively, “Receiving Parties”). This authorization and direction shall apply to all Accounts.
